Unpermitted drug and psychotropic substance use harms health, and their illegal distribution is prohibited by law and punished accordingly. Foma Bessonov returns to his hometown Kostugai to sort out family matters. In the nursing home where his grandfather lived, he finds a mysterious manuscript about Rita, who is drawn into events of the Civil War. Rita also lives in this institution and asks Foma to become her helper instead of his grandfather, promising money in return and healing from her мучительного кашля with the help of a mystical substance.

Soon Foma notices strange things in the boarding house and in the town: the elderly residents of the nursing home are involved in a bloody cult that selects its leader every year.
